ALL clients must fill out the waiver below prior to any engagement with a behavioral consultation or a training class. This includes Zoom lessons or consultations.

Which one depends on what services you are seeking, and the emotional state of your dog at the time you are filling in the form.
If you are interested in a training class (scent work, agility, basic manners, loose leash walking with just pulling, etc), zoom training session for basic manners or skills (sit, down, stay), tricks, or similar skills based tasks, and your dog is likely going to be comfortable in a training environment (like a class, or your home), and a service provider does not have to take any/many mitigating steps to keep himself/herself safe in the environment please make sure you fill out the below intake for TRAINING:
If your dog is experiencing reactivity, “over-reactions”, aggression, shut down behavior, has bitten a person or other animal, you are seeking help for unpredictable behavior, barking at every truck that goes by, growling, etc., and/or the provider needs to be made aware of what exactly to do to keep everyone, including himself/herself safe, or the dog from having an incident (peeing on the floor from fear, lunging and growling at the provider, dog is uncomfortable with new people, etc), please fill out this intake form for BEHAVIORAL SUPPORT, even if you wish for a Zoom consultation.
